A TeleHealth program provides two way communication between the patient, the family/caregivers, and agency clinical staff. TeleHealth allows for more frequent assessments, for education and for prompt response and intervention to potential problems. TeleHealth is not intended to replace all or most in-home face to face visits, but rather to ensure continuity of care and increased access to care for all patients. TeleHealth visits are a supplement for providing intermittent education, monitoring, assessment, and evaluation services between on–site visits.
TeleHealth is appropriate for a number of patients. Care can be provided to those who are homebound, who have suffered a recent change in health condition, or who have recently received an acute diagnosis. Ideal candidates for the TeleHealth program include:
