What Services does EverCare at Home offer?

EverCare at Home offers skilled nursing, physical therapy, occupational therapy, speech language pathology, telehealth, a joint replacement rehabilitation program, at home health aides, nutritional services, medical social services, and medical supplies.

Usually, your doctor will recommend any of the services listed above. Home care is an essential service that is provided in your home often after a hospital event or nursing home stay. However, anyone can make the referral to home care, meaning your doctor, your family, or yourself.

Your visit frequency can vary greatly based on your medical needs. This could be as little as one time per week, three times per week, or daily. It really depends on the Plan of Care that is developed for your diagnosis. The frequency is decided by the nurse or therapist who will collaborate with your doctor after you first home care visit.

After you choose EverCare at Home to be your home care agency, a nurse will visit to conduct an assessment. An assessment is a physical examination which will include questions regarding your medical history and medications that you take. This usually takes one to two hours. The goal of this assessment is to document your needs and make recommendations to the doctor regarding a specific plan of care developed for you.

After we visit you in your home, the nurse or therapist will consult with your doctor to establish a plan of care that meets your healthcare needs and works toward improved health outcomes.

It depends.  Payor sources can be private pay, medicare or medicaid, or it can be your commercial insurance.  We also provide charity care to those who qualify.
