August 1996: Health Advantage Plan [Managed Long term Care Plan] incorporated to provide daily living services at home. Licensed Home Care obtained in 1995; Elant at Home opened 2005.
November 2000: Lifestyle Concepts, LLC incorporated to provide consulting, developing and management services.
December 2001 with: Arden Hill Senior Health System officially changes name to Elant, Inc., Arden Hill Life Care Center changing to Elant at Goshen April 2002. Use of Elant name became system-wide to. Arden Hill Services becomes Elant Foundation December 2001.
January 2002: Health Advantage Plan changes name to Elant Choice
September 2005: Elant Academy established for in-house training and education
July 2008: Expansion of Elant Choice into Ulster and Dutchess Counties.
April 2015: EverCare Choice formally separates from Elant system with own Board of Trustees for Managed Long Term Care Plan, Certified Home Health Agency [CHHA] and Social Day Care services.

July 2015: EverCare At Home receives CHHA Certification from Center for Medicare and Medicaid Services [CMS]
August 2016: CHHA starts Fast Track Program
October 2016: Caregiver Support Group begins; CHHA Open House
EverCare hires first in-house Chief Medical Officer
November 2017: New Orientation Training Program includes OASIS Training
January 2018: CMS Conditions of Participation (CoP) becomes effective EverCare At Home growth continues to over 1,000 members, patients, clients
May 2018, 2021, 2024: CHHA Awarded Community Health Accreditation Partner, Inc. [CHAP]
March 2019, 2022: EverCare Awarded Health Ethics Trust [HET] Compliance Certified Program designation

Our Certified Home Health Agency (CHHA) provides skilled nursing, physical, occupational, and speech therapy, as well as personal care aides and nutritional support. We work under a physician’s plan of care to help individuals recover from illness, manage chronic conditions, or age safely in place—all while maintaining dignity and independence.
EverCare Life
A warm and welcoming Social Adult Day Program designed to enrich the lives of older adults through daily activities, companionship, and support. This program helps reduce isolation, promotes cognitive and physical activity, and provides respite for caregivers, giving families peace of mind.
